If you are a parent/guardian, and suspect your child might have ADHD, I first schedule a meeting with the parent(s)/guardian(s), to discuss presenting concerns.
If you would like formal testing, it is important to contact your insurance company, to find out if there is reimbursement for testing. Sometimes, when there is, there are limitations on the amount of time allotted for this. Understand you may have to pay out-of-pocket, depending on your insurance company. This often involves a prior authorization for testing.

(If you are unable to get a Teacher Rating prior to the first appointment, that is ok. Do not worry about scoring the assessment.
If the child is with more than one primary caregiver, in different environments, please submit one parent assessment for each adult provider.
If the child has more than one teacher, get a teacher assessment from someone whom you feel knows the child best and if possible, has known the child for at least three to six months. (This can also include classroom aides.)
